Case management coordinators are responsible for assisting our clients through a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet your individuals needs through communication and available resources to promote quality cost effective outcomes.
The Vocational Training program is designed to provide on the job training. Our participants require the extra time and support offered to obtain vocational skills necessary for supported employment upon completion. The process includes the following:
Our counselors assist with developing a written plan that outlines what career goals you want to accomplish and what steps you can take to meet those goals. As a result, you are able to:
• identify, organize and plan your career.
• personalize the plan that best reflects your career aspirations, whether you want to plan for professional development, promotional opportunities or retirement in the next few years.
Our employment counselors work with individuals to assist them in making the most effective career decisions. Most decisions are based on the individual’s “dream job confession,” Plan B to the “dream job,” current education level, past work experience, past criminal history, and past work recommendations. After assessing this information a plan is put into action with the assistance of community partnerships for possible job opportunities.
The Healthy Living Program promotes Health and Wellness through education of Diabetes, Heart Disease, Colon Cancer, Prostate Cancer, Sexually Transmitted Diseases and other diseases and health concerns that are prominent in low socioeconomic communities.
This program provides an array of services to Veterans and their families such as:
• Case Management
• Individual Counseling
• Group Counseling
• Family Counseling
• Housing/Community Referrals
• Substance Abuse Counseling
• Anger Management Counseling
